Medical examinations are required for all individuals applying for U.S. green cards or citizenship, per the United States Citizenship and Immigration Services (USCIS). Only physicians designated as civil surgeons by USCIS are able to complete an immigration physical, and AFC Urgent Care Grand Rapids has the physicians onsite to handle the needs for applicants of all ages.

Please bring a valid form of photo ID, and a copy of your vaccination history, as well as a translation of the documents if your vaccination reports are not in English. If you do not have these records, our physicians can perform a blood test to prove you’ve been immunized previously.
